How much longer will I continue to receive monthly calls from my Carbon RPM Care Coordinator?

You will receive your final call from your Carbon RPM Care Coordinator in January to review how you’re doing and to discuss the transition to our hypertension logging feature in the Carbon Health app.

‍## When will I receive the new cuff? What do I do with the old cuff?

You can expect to receive the new cuff in mid February. You can keep your original cuff as a back-up.

‍## Will the new cuff automatically share my blood pressure readings with the Carbon Health app?

When you receive your new blood pressure cuff, it will need to be paired with the Carbon Health app for data to be shared automatically. Please follow the instructions provided in the box to pair your new cuff with the Carbon Health app. If you have any trouble pairing your device, you can call our support team at 888-851-6481 from 7:00 am to 7:00pm Pacific.

How do I get access to the Carbon Health app?

What will happen if I continue to take blood pressure readings with my current blood pressure cuff?

Beginning January 31, 2023, any blood pressure readings taken on your current blood pressure cuff will not automatically transfer to the Carbon RPM app. The Carbon RPM app will be disconnected and you will no longer have access to it.

We encourage you to manually log your blood pressure readings in the Carbon Health app until you receive the new blood pressure cuff in the mail.

Will I continue to be charged for RPM?

There will be no further monthly charges. You will no longer receive monthly calls from your Care Coordinator. There’s no cost associated with logging your blood pressure readings in the Carbon Health app.

Do I have to accept a new cuff if I don’t want to?

If you would prefer not to be shipped a cuff, please let your Carbon RPM Care Coordinator know this and we will note in our system that you have requested not to be shipped a new blood pressure cuff that has been designed to share your blood pressure readings wirelessly with the Carbon Health app.

‍## Will my provider have access to my blood pressure readings?

Yes, your provider will be able to see your readings during a virtual or in-person visit.

When will my historical blood pressure readings be viewable in the Carbon Health app?

Our team will be working on transferring your historical blood pressure readings from the Carbon RPM app to the Carbon patient app. This will take some time and will not happen immediately. Please allow 60 days for the records to show in your Carbon patient app.

Once your historical blood pressure readings are transferred, you’ll be able to find it under “Documents” in the Carbon patient app.

‍## Will my Provider or Care team be monitoring my blood pressure when I am not meeting with them?

No, your Provider and Care team will not be monitoring your blood pressure remotely. If you ever have a question or concern about a reading outside of your appointments, please message us via Carby within the Carbon Health app and we will respond.
